Home is where recovery happens — if the support is there
In nearly three decades of helping families through hospital discharges, we've seen the pattern the data confirms: nearly one in five older adults is readmitted within 30 days — and much of it is preventable. Working in both hospitals and home care, our owner, Scott, saw it all too often: a hospitalization leads to confusion, confusion leads to a fall or missed medications, and unchecked, that leads right back to the hospital. Hospitals work hard on discharge plans, but a plan only works if someone is there to carry it out.
A caregiver interrupts that cycle. From the bedside through the ride home and the first weeks of recovery, the caregivers we refer help with the daily tasks recovery demands — keeping your loved one safe and supported while their strength returns. Many families arrange care for just the first few weeks; others discover a little ongoing help is what makes staying home work long-term.
Support during the stay — not just after it
Hospitals and rehab facilities are stretched thin, and call buttons don't replace a person in the room. Families often arrange private caregivers to sit bedside — providing companionship, an extra set of eyes, and a hand with comfort — right in the hospital or rehab facility. We help wherever home is at the moment. And when discharge day comes, the caregiver your loved one already knows can be the one who brings them home.
How Caregivers Help — In the Facility and at Home
Every situation is different, so caregiver services are built around your loved one's needs. A caregiver referred by Abby Services can assist with:
Signs It May Be Time for Extra Support
Discharge planning moves quickly, and families often don't realize what's needed until they're already home. Extra support makes sense if your loved one is:
- Alone in the hospital or rehab while family juggles work and life
- Coming home after surgery, a fall, a stroke, or a hospital stay
- Being discharged "with assistance" — and family can't be there around the clock
- Living alone, or with a spouse who can't physically help
- Facing stairs, bathing, or meals they can't yet manage safely
- Anxious or uncertain about being home safely — or you're the one lying awake worrying
- Someone you can't be with — because you live out of town or can't take leave from work
If any of this sounds familiar, our team is happy to help you find the best path forward — before discharge day, or after it.
“We used Abby Services after my mother discharged from the hospital/rehab facility. Abby Services was quick to respond to all of my needs and very informative throughout the entire process… The caregiver we were matched with was compassionate, on-time, and overall a joy to have helping my mother.”

Hospital & Rehab Support Questions, Answered
Yes — bedside support is one of the most common requests we get, especially from out-of-town family. Hospital stays often bring acute confusion, especially for those with dementia — our owner, Scott, saw it firsthand working in the ICU at Lee Memorial in Fort Myers. A familiar, consistent presence at the bedside makes a real difference: reassurance, watching for changes, and keeping your loved one from being alone in a disorienting place. We help wherever home is at the moment.
Often, yes — this is one of the most common situations we help with, and the more advance notice we have, the better. Call us as soon as you know discharge is coming, even if the date isn't final. The sooner we have your documents, payment arrangements, and caregiver preferences in place, the faster we can make a match. In urgent situations, care can be arranged in as little as an hour.
What the hospital arranged is most likely covered by Medicare — nurses and therapists ordered by the physician, visiting for short appointments a few times a week to handle medical needs. But Medicare has limits on what it covers and provides, and families often need or want more support than that. That's where a private caregiver comes in, paid for privately, covering everything in between: meals, medication reminders, bathing, supervision, and someone actually there. Most recoveries benefit from both.
Many families request transportation home as part of the arrangement, and many caregivers provide it — either driving the client's vehicle or their own. Like all services, transportation is something you and your caregiver agree on directly, so let us know it's a priority and we'll work to match you with a caregiver ready and willing to assist with your specific needs.
There's no magic number — it varies with the recovery, and that's where the flexibility Abby Services offers shines. Many families arrange support for the first two to six weeks and taper off as strength returns; others find a little ongoing help is what makes staying home sustainable. We recommend scheduling the support you think you'll need — you can scale back once you see how recovery progresses. You're always in charge.
Medicare is limited in what it covers. It will often include the home health services your doctor orders — nursing visits and therapy — but generally not private caregivers for daily support. Most families use private pay or long-term care insurance, and veterans may qualify for aid. Call us, and we'll help you understand the options for your situation.
We're a matchmaking service, and we like to say you don't marry the first person you go on a date with. While we work hard to find the best fit the first time, that isn't always possible — you can request a different caregiver at any time, no hard feelings, no hassle. The right personality match matters, and your feedback helps us identify better candidates for you. We'll keep working with you until you find the right fit.
Often, yes. Call as soon as a discharge date is mentioned. Many families have a caregiver lined up within 24 hours during the work week once documents, payment arrangements, and preferences are in place, and if discharge comes suddenly, we'll prioritize your search.
